Course Exam Request

UWDMC is no longer accepting new candidates in the Diploma program or new students in courses.  Those already registered and active will be able to complete their studies.

When you are ready to take course final exam, select a proctor who will monitor the exam process. The proctor can be a colleague, supervisor, librarian, or other individual of your choice at your location. Complete the Exam Request Form provided in your course welcome email or at the link below and email it to UWDMC at dmc@epd.wisc.edu.

Course Exam Process

Typically we mail or send via DHL delivery service a paper copy of the exam directly to your proctor. The proctor and you agree on a time and place to take the exam. The proctor verifies your identity and then stays in the room with you while you complete the exam. Both you and the proctor sign and date the completed exam. The proctor then returns the exam via mail or other delivery service to UWDMC for grading. We try to grade the exam within few days of receiving it and notify you of the results via email. You will receive a certificate and letter acknowledging that you’ve completed the course and earned Continuing Education Units (CEU).

The exam is a combination of multiple choice, true/false, and short essay questions. It typically takes one to two hours to complete the exam, but there is no time limit. You are not allowed to bring the study guide or other course materials in to the exam room.

For special situations only:  If postal mail or delivery service to your location is problematic, please contact UWDMC about other exam delivery options.
